What is Back Pain?

Back pain is one of the most prevalent musculoskeletal complaints worldwide. It affects approximately 90% of Americans at some point in their lives and is the second most common reason for medical consultation after the common cold. Back pain is not a disease in itself but rather a symptom that can arise from many different underlying processes involving the muscles, bones, discs, nerves and ligaments of the spine.

The spine consists of 33 vertebrae stacked in a column, cushioned by intervertebral discs and stabilised by a complex network of muscles and ligaments. Any pathology affecting these structures can produce pain ranging from mild discomfort to debilitating radicular nerve pain. Back pain may be classified as acute (less than 6 weeks), subacute (6–12 weeks) or chronic (more than 12 weeks).

Causes of Back Pain

Primary Spinal Causes

  • Muscle and ligament injury: The most common cause. Overstrain, improper lifting or sudden twisting can tear muscle fibres and ligaments, causing acute back pain.
  • Lumbar spondylosis (disc degeneration): As discs age, they lose moisture in the central portion (nucleus pulposus), causing reduced disc height, stiffness and pain. This degeneration is the most common structural cause in adults over 40.
  • Disc herniation and radiculopathy: Bulging or herniated discs press against nerve roots, causing radicular pain (sciatica), numbness and weakness in the limbs.
  • Spinal infections: Osteomyelitis and tuberculosis of the spine (Pott’s disease) can cause deep, boring spinal pain with constitutional symptoms.
  • Cauda equina syndrome: A medical emergency caused by direct compression of the spinal cord, potentially causing paralysis, bladder and bowel dysfunction.

Burning back pain worse at 3–4 am. Cervico-brachial neuralgia aggravated by touch. Must sit up in order to turn in bed. Suited to irritable, ambitious individuals who overwork and have sedentary habits with poor posture.

Kali Carbonicum

Sharp, stitching pains. Back feels stiff, weak and burning with perspiration over the back. Pain from hip to knee. Pains aggravated in a cool climate. Specifically suited to backache occurring during pregnancy.

Guaiacum

Chief action on fibrous tissue; adapted to rheumatism and gout. Stiffness in the back and lower limbs, stiffness of the neck and sore shoulders. Left-sided sciatica. Pains aggravated by motion, cool climate and touch.

Gaultheria

Suited to sciatica and neuralgic pains. Neuralgia associated with gastralgia and rheumatic complaints. Useful for burning, shooting nerve pains along the sciatic distribution.

Gnaphalium

Sciatica associated with numbness. Right-sided sciatica. Back pain better from resting on the back. Alternating pain and numbness is a keynote. Cramping pain in the calves.

Causticum

Stiffness between the shoulders, dull pain in the neck, left-sided sciatica with numbness. Contraction of tendons. Unsteady walking. Weakness of muscles and slow paralytic progression in long-standing nerve compression.
